Rates
My standard fee for a 40-60 minute individual therapy session is $200. Initial Appointment fee is $250.
Missed appointments and cancellations that occur with less than 48 hour notice prior to your scheduled appointment are charged $150. Please note that insurance carriers do not cover missed or late cancellation fees.
Payments
I accept cash, check, and all major credit cards as forms of payment.
Good Faith Estimate
Under the No Surprises Act, you have the right to receive a "Good Faith Estimate" explaining how much your medical care will cost for any non-emergency items or services. Insurance Information
I'm currently in network with Optum (United Behavioral Health Care).
At this time, if you have any other insurance plan, I am considered "out of network." for all insurance carriers. If you have a PPO and I am out-of-network with your insurance carrier, until your benefits are verified and we have discussed your coverage, you would pay the full fee directly to me at the time services are rendered. Application of any co-pays or co-insurance requires that coverage be verified in advance of your initial appointment. If you chose to submit a claim to your insurance company, I can provide a monthly superbill.

Depending on your current health insurance provider or employee benefit plan, it is possible for services to be covered in full or in part. Please contact your provider to verify your coverage. I'd recommend asking these questions to your insurance provider to help determine your benefits.
Does my plan provide 'Out-of-Network' benefits for mental health?
Do I have an out-of-network deductible? Does my deductible cross over or is it serparted from my in-network deductible? If so, what is it and have I met it yet?

What percentage of that allowed amount is covered? (e.g., my fee is $200, but your insurance companies allowed amount is $150 and they have 60% coverage, they will only get back $90).
